On May 29, Tzu Chi USA is administering Moderna’s COVID-19 vaccine.
To get yours…
- make an appointment online by Friday, May 28 using the form below OR
- walk in. If you do so, we recommend you arrive early while supplies last.
NOTE: If you had registered for our previous vaccination event on 5/1, please arrive as a walk-in from 8am – 12pm, making sure to arrive earlier than later.
For appointment holders, please arrive at your allotted time at the Tzu Chi Education Campus, located at 1920 Brea Canyon Cut-off Road, Walnut, CA 91789.
For walk-ins, please arrive at the same location between 8am – 12pm.
Bring any form of photo ID and, if it is your second dose*, your vaccine card.
For first dose recipients, instructions will be provided on site to receive your second dose at a later date.
For second dose recipients, you must have received Moderna’s COVID-19 vaccine as your 1st dose. No exceptions.
Tzu Chi USA is administering up to 1,000 doses of Moderna’s COVID-19 vaccine on Saturday, May 1. You may make an appointment for your first or second dose* using the following form.
Once reserved, please arrive at your appointment time at the Tzu Chi Education Campus, located at 1920 Brea Canyon Cut-off Road, Walnut, CA 91789.
Bring any form of photo ID and, if it is your second dose*, your vaccine card.
*In order to qualify, you must have received Moderna’s COVID-19 vaccine as your 1st dose. Due to supply, this brand is our only offering at this time.
